You may use this page to create a Tax Credit Receipt request. Fill in the appropriate form fields and click the "Go To Printable Page" button. Print the second page and send it, along with your payment, to the address provided. Ajo Unified School District will mail a Tax Credit Receipt to you at the address you provide. You may leave the "Contributor" information blank. In this case the receipt will be sent to the person and address as it appears on your check.
